The Qantas Airways Boeing 747-438 VH-OJB 'City of Sydney' was involved an a near head-on collision with an U.S.A.F. Lockheed C-5A Galaxy, over Thailand at night and in cloud. It was estimated that the aircraft passed within 15 metres of each other.
